"Send form to what e-Mail address?:" Type in the e-Mail address where you want the form to be delivered when someone clicks Send.

"Do you want to CC any more e-Mail addresses?:" If you want to send Courtesy Copies or duplicates of the form submission to any additional inboxes besides the one you gave above please type the e-Mail addresses here with a space between each one, otherwise leave blank for no. Maximum 10 allowed.

"Type your autoreply message? (optional):" If you have a short generic message you would like to include as an autoreply for every form submission then please type it here, otherwise leave blank for no autoreply.

"Redirect to URL after submit?:" If you would like to redirect a person after they submit the contact form to a certain web page address please type it here, otherwise a standard "Thank You" page is automatically generated.

"Would you like to add any additional file types?:" Currently the Form's file upload feature supports (txt, gif, jpg, jpeg, zip, doc, png, pdf, rtf, html, docx, xslx) You may add up to 10 more file types of your choice at the time of ordering, please specify the correct extensions.

There are certain questions you must answer for us to build you a complete, ready to use out of the box Flash Contact Form! We only deliver complete forms with simple instructions that can be used immediately on any web site that supports PHP 4.3.11 or higher.

All forms are customized based on the information you submit, please type all information accurately and spell checked. The Form is then built for you after ordering and ships via e-Mail within 24 hours, usually less.

Your form will be delivered inside a Zip file which you must extract to a folder. You must copy & paste the provided embed code to your web site. You must upload the entire contents of this Zip file to your web server in the same directory you want the Form to appear. For example, if you pasted the embed code into "contact.html" then all the required files inside the Zip must be uploaded in the same directory as "contact.html".

You will need computer software to "unzip" the Form. If you don't already have this kind of software on your computer I would recommend you download CAM UnZip since it is fast and free.

Although there is no code to setup involved with the Flash Form, it is up to you to make sure your web hosting server supports PHP 4.3.11 or higher. Otherwise it will not work.
